Every Bupa Aged Care home is part of a Wellness Hub - a team of healthcare professionals working together to provide you with quick and easy access to a range of health services including physiotherapy, dietitian services, speech therapy, wound consultants, all coordinated by our Nurse Practitioners and clinical leaders.
Our Wellness Hub team works together to help ensure you receive timely referrals, assessments , and treatment plans. We also offer support for telehealth consultations and the implementation of personalised care programs.

Your aged care team is there to take an active interest in your health and wellbeing so that, together, we can help make life happier and healthier. Get to know some of our dedicated healthcare professionals that are available in your care home or through telehealth consultations.
Nurse Practitioner
Leads the Wellness Hub team and collaborates with nursing teams in the home to help ensure resident care is optimised.
Care Manager
Is the clinical leader in your care home coordinating your health needs with the Wellness Hub team and assists residents in accessing optical, dental, and audiology professionals (additional costs may apply for these services).
Speech Pathologist
Works with residents to assess and manage swallowing and speech difficulties and developing care plans for ongoing care.
Physiotherapist
Works with residents to help maintain physical function and mobility through appropriate care planning, targeted exercises, and reablement opportunities.
Occupational Therapist
Assess residents' functional abilities, evaluate the need for adaptive equipment such as mobility scooters, and create plans to help enhance performance in daily activities.
Bupa Dietician
Assess nutritional needs and create personalised meal plans, including modified texture diets. They address issues such as unintended weight loss and pressure injuries while educating residents on healthy eating.
Bupa Wound Consultant
Assess and provide treatment recommendations and develop personalised care plans to help promote wound healing and help prevent complications.
Carer Therapist
Facilitates telehealth appointments and implements care plans provided by clinicians to support reablement.
Registered Nurse
Delivers nursing care to residents, supervises care workers, and facilitates appointments with the Bupa Wound Consultant.

What services are available through the Wellness Hub for residents?
The Wellness Hub features a Nurse Practitioner-led multidisciplinary team that collaborates virtually to provide comprehensive and personalised care for our residents. Based on your needs a range of health services are available, including Nurse Practitioner reviews, physiotherapy, dietitian consultations, speech therapy, occupational therapy and wound consultation. Each resident receives personalised assessments and care plans tailored to their specific health needs and in partnership with their primary care provider.
How can I access the Wellness Hub?
Residents are referred to the Wellness Hub by the clinical team within the care home. The Care Manager will also guide you through the available services and help coordinate your care with the appropriate health professionals.
Is the Wellness Hub run via telehealth consultations?
The Wellness Hub offers support both in person and with telehealth consultations, allowing residents to connect with health professionals in a variety of ways. This provides flexibility and ensures that residents can receive care quickly using the most appropriate method. Visit method may be dependent on your care home location.
Are there any costs associated with using the Wellness Hub?
No, there aren't. However, some additional costs may apply for specialist visits outside the Wellness Hub, such as those for optical, dental, and audiology professionals or other specialist health services. It's best to check with your Care Manager for specific details regarding costs.

Your Care Manager acts as the clinical leader within in your care home, coordinating your health needs and assisting you in arranging visits from various health professionals. They help ensure that you receive comprehensive care tailored to your individual requirements.
